CRIMINAL HISTORY BACKGROUND CHECK PROCEDURE

AUTHORITY: Utah Code 32B-1 Part 3

Criminal history background checks are required for all owners and persons employed to act in a supervisory or managerial capacity for a package agency, licensee or permittee.

If the owner is a partnership, corporation or limited liability company, a criminal history background check is required for all partners, managing agents, managers, officers, directors, or any stockholder who holds at least 20% of the total issued and outstanding stock of the corporation, or a member who owns at least 20% of the limited liability company.

Each individual who falls under the description above shall consent to a criminal history background check and shall deliver the listed documents and fees to the Department of Alcoholic Beverage Services.

New DABS license applicants must submit fingerprints either prior to submitting their application or with their application. Please plan ahead for fingerprinting services in order to meet your deadline before the 10th of the month.

Each individual who falls under the description above shall consent to a criminal history background check and shall deliver the below documents and fees to the Department of Alcoholic Beverage Services:

• $42 submitted to DABS for each person fingerprinted regardless of where fingerprinting is completed
• Fingerprints - either through a live scan service or a completed FBI fingerprint card
• A signed Informed Consent & Waiver form

• You must register in the DABS Learning Portal
  • To ensure all paperwork is accurate and completed, please use a desktop or laptop computer
  • Each individual obtaining a background check must register separately in the learning portal, prior to payment
• Select "Fingerprinting & Background Checks"
• Pay the $42 fee
